IF YOU HAVE ANOTHER INSURANCE THAT IS NOT LISTED ABOVE, DON’T WORRY. WE ARE ABLE TO SUBMIT CLAIMS TO MOST INSURANCES, EXCEPT TENNCARE/MEDICAID/MEDICARE. YOU CAN CALL THE CUSTOMER SERVICE NUMBER ON THE BACK OF YOUR INSURANCE CARD TO VERIFY A PROVIDER’S NETWORK STATUS.
PLEASE NOTE: PAYMENT IS DUE AT THE TIME SERVICES ARE RENDERED.
As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies respond to the claim within four to six weeks. Any remaining cost is your responsibility.
